Results of AAFP Demonstration Project
The AACH focus on personal awareness and improved leadership and team building skills was recently reinforced by an important initial summary of the results of the AAFP National Demonstration Project. The evaluation team made the following observations about attempts to transform traditional practices into medical homes.*
AACH Board approved joining the Patient Centered Primary Care Collaborative as a member organization. As part of its commitment to improving the quality of partnerships between practitioners, patients and healthcare teams, AACH believes that redesign of primary care practice, to allow increasing methods of communication and improved coordination of care, is an essential component of an improved healthcare system.
